Referrals

All patients must obtain a physician prescription for physical therapy AND private health insurance referral (if required) prior to initial visit.  Patients are responsible for health insurance referrals throughout course of physical therapy. Please contact us for your specific insurance provider number.

 

Patients are also responsible for insurance copayments and deductibles, based upon their specific insurance plan.  Copayments are due each physical therapy session.
